Healthy scalp perfusion supports follicle function and nutrient delivery, which matters particularly for follicles that are already under hormonal stress.

There is also a hormonal dimension worth examining. Moderate, consistent aerobic exercise is associated with improved insulin sensitivity and reductions in systemic inflammation, both of which play supporting roles in a healthy hormonal environment. Some research suggests that excessive DHT elevation can be compounded by metabolic dysfunction; improving metabolic health through exercise may therefore create a more favorable baseline for finasteride to work against.

Alcohol: The Factor Most Men Underestimate

First, finasteride is metabolized in the liver by cytochrome P450 enzymes. Alcohol is also processed hepatically, and chronic or heavy drinking increases the liver's metabolic workload while impairing enzyme function over time. Higher estrogen relative to testosterone can affect the hormonal balance that finasteride is attempting to modulate, and may contribute to side effects such as gynecomastia that are already a documented, if uncommon, risk of the medication.

None of this means that a glass of wine or a weekend beer will derail your treatment. The concern is with regular heavy consumption — generally defined by the CDC as more than 14 drinks per week for men. At that level, the hepatic and hormonal effects become clinically relevant. If your drinking patterns fall into this range, it is worth discussing openly with your physician, both for the sake of your finasteride outcomes and your broader health.

Hydration and Its Overlooked Role

Chronic mild dehydration is more common among American adults than most people appreciate, and its effects extend well beyond thirst and fatigue. From a pharmacological standpoint, adequate hydration supports proper kidney filtration, which plays a role in clearing finasteride metabolites. It also maintains blood volume, which influences the consistency of drug absorption and distribution.

For hair health specifically, dehydration affects the nutrient delivery capacity of blood to peripheral tissues, including the scalp. While this is unlikely to override finasteride's primary mechanism, it represents a low-effort variable that is entirely within your control. The standard recommendation of approximately 3.7 liters of total daily fluid intake for adult men (as outlined by the National Academies of Sciences) is a reasonable target, with additional intake warranted on days involving significant exercise or heat exposure.

Building a Lifestyle That Works With Your Treatment
